MAMSI Member Registration- Registered User Agreement
Please read this registered user agreement carefully. You may not use the MAMSI member online system unless you first accept the terms and conditions of this agreement by electronically clicking the button labeled "I accept ." By accepting the terms of this agreement, you are authorized to use the system during the term of this agreement and subject to its terms and conditions. MAMSI reserves the right to terminate this agreement at any time for any reason.
eMAMSI ON-LINE SERVICES AGREEMENT REGISTERED USER AGREEMENT This eMAMSI On-Line Services Agreement (the "Agreement") is provided for the purpose of granting you access to eMAMSI, the Iinternet-based member services information system of Mid Atlantic Medical Services, Inc. ("MAMSI") and its subsidiary health plans, including MD.D.-Individual Practice Association, Inc,. ("M.D. IPA"), Optimum Choice, Inc.(R) ("OCI"), Optimum Choice of the Carolinas, Inc. ("OCCI"), and MAMSI Life and Health Insurance Company ("MLH"). M.D. IPA, OCI, OCCI and MLH are referred to as the "MAMSI Health Plans.". eMAMSI consists of online claims status, the Healthwise(R) Knowledgebase health information service resource and other services that we may make available to you from time to time through the member section of our Web site (the "eMAMSI Services"). You should carefully read the following terms and conditions before using this software. BY CLICKING I ACCEPT THIS AGREEMENT, YOU AGREE TO BE BOUND BY THE TERMS AND CONDITIONS SET FORTH BELOW. IF YOU ARE NOT WILLING TO BE BOUND BY THESE TERMS AND CONDITIONS, YOU MAY NOT ACCESS THE eMAMSI SERVICES. 1. Registration Terms. This Agreement governs your use of eMAMSI. In order to access eMAMSI, you must first register yourself as a qualified user. Registration means the following: a. You must complete the registration form (which asks you to supply information such as your member number, user name and a password). b. You must submit the completed registration form to MAMSI. c. You accept all of the terms of the eMAMSI Registered User Agreement. 2. 2. Safeguard your User ID and password. You must register on the MAMSI Web site using a User ID and password assigned to you through the MAMSI Web site. You will need your User ID and password in order to use the eMAMSI services. It is important that you safeguard your User ID and password from theft and to prevent unauthorized users from accessing your health information. By accepting this eMAMSI Registered User Agreement, Yyou agree that you will protect your User ID and password. that you will need in order to use the eMAMSI Services and safeguard them from theft and misuse by others. 3. Grant of License. MAMSI hereby grants to you (the "User") a non-exclusive, non-transferable license to use the eMAMSI Services. User shall not use the eMAMSI Services except as expressly permitted by this license. You may use the eMAMSI Services only for legitimate personal use. You may not sell, license, assign or transfer this license or your rights under it to any other person or entity. You may not reproduce, modify or distribute any of the eMAMSI Services. You must be an enrolled member of a MAMSI Hhealth Pplan in order to access the eMAMSI Services. You must be at least 18 years of age to access the eMAMSI Services. You are entitled to download and save the health information contained in the eMAMSI Services for your personal use only. 4. Disclaimer. a. Health information resources. The Healthwise Knowledgebase information eMAMSI Services accessed through eMAMSI Services on this Website is provided for informational purposes only and is presented in summary form only to provide a general understanding of health care topics. The information should not be considered complete and should not be used in place of consultation with your health care provider. With respect to Healthwise link on the Web site, neither Healthwise nor MAMSI assumes any responsibility for any circumstances arising out of the use, misuse, interpretation or application of any information or other materials provided on or through eMAMSI. All information contained on this Web Site is provided "as is" and without warranty. Any express or implied warranties are explicitly disclaimed, including, but not limited to warranties merchantability, fitness for a particular purpose, or non-infringement. MAMSI also makes no warranty as to the reliability, accuracy, timeliness, usefulness or completeness of the information accessed through the eMAMSI Services. b. On-line claim status and other benefit information. eEMAMSI also provides on-line status of claims information regarding claims that you may have which you have submitted filed to one of the MAMSI Health Plans as well as other benefit information.. . . . eMAMSI's on-line claim status information is not intended to replace or modify your plan documents or other member materials, and may not be a substitute for obtaining additional information regarding how we processed your claim. We strive to provide accurate claim status information. If the need should arise, you can still contact your employer benefits department or our Member Services Department for more information. c. No warranty; protecting your computer. All information contained on this Web sSite is provided "as is" and without warranty. Any express or implied warranties are explicitly disclaimed, including, but not limited to warranties of merchantability, fitness for a particular purpose, or non-infringement. MAMSI also makes no warranty as to the reliability, accuracy, timeliness, usefulness or completeness of the information accessed through the eMAMSI Services. MAMSI and Healthwise cannot and do not warrant against errors, omissions, delays, interruptions or losses, including loss of data. It is solely your responsibility to take any and all precautions necessary to ensure that the eMAMSI Services is free of any viruses, worms, Trojan horses, or other destructive items or bugs. In no event will MAMSI be liable to you or any other party for any actual, , indirect or consequential damages that may arise as a result of your use of the eMAMSI Services, even if MAMSI or any of its subsidiaries or affiliates have been advised of such damages or losses. 5. Intellectual Property. Title to all copyright and other intellectual property rights in and to the eMAMSI Services, and any copies of the eMAMSI Services or the health information, are owned by Healthwise, Incorporated. By using the eMAMSI Services, you agree not to assert any intellectual property rights in or to the eMAMSI Services. Therefore, you must treat the eMAMSI Services like any other material protected by copyright and other intellectual property rights. eMAMSI, MAMSI, Optimum Choice, Inc., and "Your Life. Our Mission." are service marks of Mid Atlantic Medical Services, Inc.. 6. Modification of Agreement. MAMSI may modify this Agreement at any time, and such modification shall be effective immediately upon posting of the modified Agreement. To continue to use the eMAMSI Services, you must read and agree to modifications to the terms of the Agreement. You must register on the MAMSI Website using a User ID and password assigned to you through the MAMSI Website. 7. Term of Agreement. MAMSI reserves the right, in its sold discretion, to terminate your access to the eMAMSI Services in the event you violate the terms of this Agreement or in any manner misuse the MAMSI Web site or the eMAMSI Services.
